Here’s a look at the issue.

Former U.S. Rep. Michele Bachmann said the Biden administration is proposing amendments to the World Health Organization’s International Health Regulations that would transfer U.S. sovereign authority over health care decisions to the WHO director-general. The International Health Regulations, which are aimed at detecting disease outbreaks, allow the WHO director-general to declare a public health emergency of international concern. Member countries agree to abide by the guidelines, but the WHO does not have the power to enforce them, nor can it interfere in other countries’ decision-making processes, according to experts. 

As the WHO hosts its 75th World Health Assembly beginning on Sunday, some social media users are misrepresenting proposals the U.S. is bringing to the conference, where delegates from 194 member states convene to discuss priorities. Following the COVID-19 pandemic, the U.S. has drafted a series of amendments to a legal framework called the International Health Regulations, which define countries’ rights and obligations in handling cross-border public health emergencies. The U.S. amendments call for greater accountability and transparency in responding to such emergencies. 

But some remarks, including those by Bachmann, bloggers and conservative political commentators, are misrepresenting the proposals to claim they would take health policy decision-making powers away from U.S. officials and grant unilateral authority to the WHO’s director-general. “These amendments would transfer our health care decision-making out of U.S. hands, into the hands of the director-general of the WHO,” said Bachmann, a former congresswoman from Minnesota, while calling into a conservative radio show last week. 
The segment was posted on Facebook, where it was viewed more than 32,000 times. Bachmann went on to suggest that the same amendments would allow the director-general to impose global lockdowns and vaccine mandates, as well as force climate change policy and even gun control measures on member nations. Bachmann did not respond to a request for comment. Experts familiar with the International Health Regulations say these assertions are misleading, and the idea that the director-general could impose enforceable mandates on other countries is unfounded. 

Lawrence Gostin, a Georgetown University law professor and
director of the university’s WHO Collaborating Center on National and Global Health Law, told the AP that the director-general only has the power to make recommendations, not enact laws or otherwise dictate national policy decisions. “It is utterly untrue that the IHR would interfere with health care decisions or transfer such decisions to the WHO Director-General,” he wrote in an email.
Gostin, who also helped write the 2005 version of the IHR, cited the fact that China signed the IHR, but violated it by delaying reporting of the initial COVID-19 outbreak and later pushing back against the WHO investigation into its origins. The U.S. amendments seek to prevent this from happening, by tightening requirements for reporting information to the WHO and allowing them to conduct unimpeded investigations, among other changes.

Dr. David Freedman, the president-elect of the American Society of Tropical Medicine
and Hygiene, who served on a WHO committee of IHR experts for a decade, reiterated that the WHO “has zero enforcement, police or punitive powers.” Further, the IHR is mostly focused on preventing the spread of infectious diseases and pandemics, he said. Climate change, gun control or even specific measures like vaccinations or lockdowns are not mentioned.
Some social media users are also conflating the IHR with a separate effort the WHO has launched to develop a global accord on pandemic prevention and response. That accord is still being drafted, but experts told the AP there’s no evidence it would cede any national decision-making powers, either. “Unfortunately, there has been a small minority of groups making misleading statements and purposefully distorting facts,” WHO Director-General Tedros Ghebreyesus said during a news briefing Tuesday, clarifying that the WHO does not override member nations’ sovereignty.

What does this mean?

Well, just think about the last three years under the COVID pandemic where we saw local governments, state governments, and our federal government make pronouncements that businesses had to close down. We were all told that we had to sit at home with the doors shut for 15 days to slow the spread.
“It is an extraordinary, breathtaking level of power that the United States government
took over its people in the last three years—closing down businesses, closing down schools, mandating that people had to get shots, whether they thought they needed them or not.”

Mandating mask-wearing, lockdowns, mandating that churches had to close, that children couldn’t go to school. This level of emergency power is what I’m talking about, Dr. Dobson, and the Biden administration wants to take that power and give it to the World Health Organization, the U.N.,” she said. She said Biden’s plan would give the U.N. agency “the power to shut us down in the United States and tell us if we have to have a shot in our arm or tell us if we have to wear masks or a number of other things that they would have the power to do.

“It would be, she said, “a platform for global governance.
“Bachmann said no threat like that has been looming in the past, as it would transfer.”
U.S. sovereignty to the U.N. “Dobson noted that the Bible described that “in the last days there will be a one-world government, a dominion that will keep people from buying or selling without the approval of a dictator.”

are liberal states Illinois, Massachusetts, Michigan, New Jersey and Pennsylvania; and
the battleground states of Missouri and Iowa.
Once a liberal State, and moving in the Conservative direction, Louisiana, will also
lose one seat. Though California did not lose a seat, for the first time in its history,
liberal Democrat-leaning California will not gain a House seat after the census.

The November election landslide by the GOP also put Republicans in full control of a number of state governments, giving the GOP an overall edge in the redistricting process, and in the fight for State’s Rights. When Obama was elected two years after the Democrats gained full Control of Congress the liberals were dancing around thinking it was the end of Conservatism. The idiots truly believed this nation was moving to the left.

The media actually began producing stories about the eventual death of the Republican Party. The reality is that this is a center-right nation, and the elections of 2006 and 2008 were the result of the fact that the sheep that usually don’t vote were also deceived by the articulate Barack Obama (and many are regretting that vote now), and the Conservatives stayed home because with moderate McCain on the ticket, they felt they had no candidate to choose.

The Conservatives that did vote, did it hold their nose, and only because they feared Obama, and they liked Sarah Palin as the Vice-Presidential choice. What is interesting is that now that the facts have surfaced, and the Census numbers are out, the truth is the exact opposite of what the liberal Democrats were saying.

In reality, liberalism is dying, Conservatism is on an ascendancy, and if Obama and
the Congressional Democrats don’t change their Marxist policies soon, it could be the Democrat Party that may wind up as just another failed ideology in the dustbin of history. Catholics are pragmatic. Until the McGovernization of the Democratic Party in 1972, they were among the most reliably Democratic voters in the nation. Since that time, they have been politically homeless, which is why both parties vie for their vote so aggressively.

Catholics are a bellwether: whoever wins their vote, generally wins the election.
By contrast, Protestants vote Republican and Jews vote Democrat. Although, Asians are another group to watch. Not only are they the fastest growing minority in the U.S.—they constitute 3 percent of the electorate—they are up for grabs by both parties. In the last presidential election, Obama captured 73 percent of their vote. But not this year: Asians voted 50 percent to 49 percent, choosing Republicans over Democrats.

This is a huge turnaround in just two years.
One final note: Some things never change. Most Americans are either high school graduates, have attended college, or are college graduates: they voted Republican,
by a wide margin. So who voted for the Democrats? High school dropouts and postgraduates.

That would make for a great “60 Minutes” segment: Why is it those who know
nothing has so much in common with those who think they know everything?

In 2016, Clinton voters were not “clustered” at the county level, but they were
“clustered” at the state level and probably at the precinct or municipality level as well.
(According to my Twitter friend Neil, only about 45% of Ohio voters were in Obama precincts in 2012, even as Obama carried the state.)  This is because “clustering” is not an inherent property of populations, but a relationship between populations and particular choices of boundaries and scales.

Under one set of lines, Democrats might be “clustered”, under another set of lines applied to the same population, Republicans might be “clustered”. If every district had exactly one person, it would be impossible for any party to be “clustered”. Biden to exempt farms in plan to eliminate stepped-up basis. 
As proposed, broader tax code changes take into consideration important saving of
step-up basis for farmers. In his first address to Congress Wednesday night, President Joe Biden will unveil his American Families Plan which calls for changing the existing tax code handling of capital gains and eliminating stepped-up basis. However, his plan will offer protections for family-owned businesses and farms who pass on the entity to their heirs.
In a call with senior White House officials Tuesday evening ahead of the speech, one official notes the plan would address the loophole in the existing tax code that allows the wealthiest Americans to entirely avoid income taxes on their gains by holding assets for their entire lifetime and passing it on to their heirs. “This reform will be designed with explicit protections so that family-owned businesses and farms will not have to pay taxes when given to heirs who continue to run those businesses,” the senior official notes.

While offering details of the tax plan, the White House official adds, “The conceptual framework for equalizing the tax treatment of capital gains and ordinary income is neither new nor novel. It was a key feature of President Reagan’s tax reform where he increased the capital gains rate to match the personal income tax rate.”  Under current federal law, an asset’s basis is the starting point for calculating capital gains taxes.
When an individual dies and passes on an asset, like a farm, to a family member, the basis of this asset is reset to the current fair market value, not the original purchase price.
Former Senate Finance Committee lead Republican Chuck Grassley of Iowa states he’s gearing up for a legislative battle over the issue of whether to allow a step-up in basis. “This tax policy helps asset-rich, cash-poor farming operations keep the family business going from one generation to the next rather than having to sell it to pay an unaffordable tax bill,” Grassley says.

When property is transferred from a deceased farmer to an heir, families use the stepped-up basis that “steps up” the current fair market value on the property to measure a capital gain and calculate tax liability. This step-up in basis reduces the capital gains tax liability on a farm passed down to the next generation because liability is based on the time of ownership of the heir, not throughout the ownership of the deceased.
Legislators hearing concerns
Commodity groups have been educating those on Capitol Hill about the potential
impact of eliminating step-up basis for farm families. In speaking to the North American Agricultural Journalists annual meeting on Tuesday, Sen. Tammy Baldwin, D-Wisc., says she continues to hear from those in the ag sector about the impact of step-up basis. 

Although in her role as agriculture appropriations chair the issue doesn’t come
under her purview, she says she will be in conversation with her colleagues on the
Senate Finance Committee to make sure they’re aware of the “unique concerns raised
among farmers about this issue” by those she’s hearing from in the agriculture industry.
Sen. John Hoeven, R-N.D., who serves as the ranking member on the agriculture appropriations subcommittee, expressed concerns about the impact of estate tax and step-up basis changes. He adds it’s important to get young people into farming and ranching and allowing for the mindful passing of a farm onto the next generation.

Hoeven says it is important for farmers and ranchers to stay engaged in the discussion
as Congress also looks at the President’s proposal and whether it changes going forward. 
Farmers and ranchers need to stay involved and talking about how important it is that they have that estate tax credit and the stepped up basis so that they can transfer the farm or ranch, because it’s not just the cost of your real estate in your land,” Hoeven says, as you also add in the incredible cost of equipment and operating costs. “So, there’s a real capital burden there, and that’s what impacts farmers and ranchers so significantly. They’ve expressed concern, and they need to stay involved as these discussions are going on.”

Negative impact
A new analysis by the American Soybean Association and the American Farm Bureau Federation also detailed the impact of the elimination of stepped-up basis. Scott Gerlt, ASA economist, and John Newton, Farm Bureau chief economist, explain that heirs facing higher taxes would incur steep costs after bringing the land to market, thereby increasing costs for everyone else in the marketplace. And, if an estate is passed on with debt, it may not be possible for the family to meet the tax obligation. Gerlt and Newton in their analysis offer perspective across the U.S. and give real examples of the damage stepped-up basis changes could cause.
“Assuming a very likely capital gains tax rate of 20%, without stepped-up basis, it’s estimated that the tax burden on farmers and ranchers inheriting cropland would be significantly larger than the cash rental income generated on the farmland.
In the case of most farm operations, the capital gains tax would take several
years of rental income to pay the tax obligation,” the analysis notes.

new report released by EY, an economic consulting firm, finds that repealing the step-up in basis tax provision would damage the gross domestic product and significantly decrease job creation.  The study was conducted for the Family Business Estate Tax Coalition, which includes almost 60 organizations representing family-owned businesses.  
The EY study found middle-class, family-owned businesses would be particularly
hard hit by the repeal. Currently, when someone inherits assets, they aren’t taxed on the appreciation that happened before they inherited them. If family-owned farms, small businesses or manufacturers are forced to pay capital gains accrued by the prior owner, they would likely face large tax bills that put the future of their business at risk.   
According to the study’s findings, repealing the step-up in basis would result in: 80,000 fewer jobs in each of the first ten years; 100,000 fewer jobs each year thereafter; and
a $32 reduction in workers’ wages for every $100 raised by taxing capital gains at death. 

To reveal the impact stepped-up basis repeal would have on family-owned farms
and ranches, EY developed a case-study based on a theoretical family-owned cow-calf operation. In this scenario—one where the stepped-up basis is no longer a tool for family-owned business to utilize when generational transfer occurs — gains are taxed at death and would result in an immediate one-time tax liability equivalent to 280% of the farm’s annual income.
National Cattlemen’s Beef Association Senior Executive Director of Government Affairs Danielle Beck says, “The EY study sheds light on the facts that we at NCBA—among others in the agricultural community—have long known. Simply put, the repeal of the stepped-up basis would have catastrophic impacts on the ability of farmers and ranchers to transfer their operations to the next generation.”

American Farm Bureau Federation President Zippy Duvall adds, “The value of many farms is tied up in land and equipment. Cash flow on most farms is much too small to pay large capital gains taxes. These taxes would cause further consolidation in agriculture with small farms more likely to be forced out of business by the tax liability.”

Biden’s quiet attack on rural America smells just as bad as manure

Big Horn County had just two outposts for health care: an 18-bed critical
access hospital and a clinic for the Crow and Northern Cheyenne tribal reservations.
Covid-19’s sweep across rural America is forcing small regional hospitals and facilities
like these to combat a virus that has stressed even the world’s biggest health-care systems. 
Rural America was the backbone of the American economy.
Our farmers feed and fuel our country, act as good stewards of our land, and have represented the American identity for generations. They value hard work, live within their means, and support our communities. They never ask for recognition, but the result of their hard work is on display in every grocery store and meat counter across the country. 

However, since the beginning of his administration, President Biden has enacted radical policies and imposed ridiculous mandates that would devastate our rural way of life and inject Washington bureaucracy into the heartland. Frankly, it should come as no surprise to the president that his poll numbers have plummeted in the Midwest, with 64 percent
of Iowans alone disapproving of his performance. 

He attacks everything that makes our rural communities thrive.
In late 2022, on the eve of the new year, the Biden administration quietly
published an expanded version of President Obama’s controversial and
destructive “Waters of the United States” (WOTUS) rule.
With no warning whatsoever, Biden and his radical Environmental Protection Agency 
subjected 97 percent of Iowa land and millions of hardworking American farmers to
arduous bureaucratic red tape, mountains of paperwork & unnecessary headaches.
Ironically, for someone who claims to advocate for all Americans,
Biden’s abysmal record of targeting America’s family farmers begs otherwise. 

As the second-largest agriculture-producing district in the nation,
I represent thousands of families who have farmed the same land for generations, providing for themselves, and also preserving their farmland, and living the American Dream. These same families – who are the best conservationists – also support clean water and take steps every day to protect our rivers, lakes, ponds, and streams in Iowa. From installing buffer strips and bioreactors to planting cover crops, our farmers exercise common sense conservation practices – that the federal government intentionally ignores – to keep our water clean and drinkable. 
Regrettably, when he had the chance to reverse course and stand up for our farmers, President Biden turned his back on the heartland, opting instead to veto a bipartisan resolution offered by House Republicans that would ease WOTUS regulations, eliminate red tape for our producers, and return local control of American farmland to our farmers. President Biden also promised that he would never raise taxes on American families making less than $400,000 a year.
 
This, of course, was another lie. 
In his nearly $7-trillion annual budget, he proposes eliminating vital tax provisions – including step-up basis – that our farmers rely on to pass their farms on to the next generation.

Step-up basis, which protects family farmers from accruing massive tax bills in the
tragic event of the owner’s passing, is vital to our rural communities and farm economy. This is the crux of President Biden’s failed tax policies. With one hand raised vowing to never raise taxes on the middle class, he’s signing family farmers’ foreclosure papers with the other.

President Biden’s veto of a bipartisan resolution to overturn his invasive
WOTUS rule and his absurd plans to tax our farmers out of existence represent
clear examples of his callous disregard for Rural America. Born, raised in rural Iowa,
I know the devastating consequences of federal overreach of higher taxes on communities.

These three states are now living with unfundable debt that will eventually bring them to failed state status. There is no way to pay off the promised debt of these states. The latest tax migration data from the #IRS shows that the exodus of taxpayers from those high-tax states continued from 2020 to 2021, with California, New York, and Illinois again suffering some of the nation’s biggest losses of people and money.

Who is going to pay for their welfare?

The problem is and Arizona is a great example, that these people that are fleeing the destruction they created via their voting choices in their destroyed cities, are doing the same thing in the place where they moved to. Sad part, the same idiots leaving those states will vote for the same communist scum that sent them running from their previous states. So if, as the Illinois’ governor claims, why did tax revenue decline by 10.9 billion?

Could it be the type of tax payers they gained or also the businesses leaving is not maintaining the balance? Math is not working. Greatest declaration coming out of NY, “We’re sorry Gov Hochul, we can’t pay you anymore”. Gee go figure?
Three states run by communists who put criminals over taxpayers. The real problem is these fucks leaving these Democrat cesspools and fleeing to Patriotic free states like Texas bring their politics with them and end up attempting to turn their new State into the same cesspool they just left. May need a continued government bailout as citizen resources diminish.

Today and for eternity, he’s in heaven without a worry about anything.
However, I celebrate his birthday with joy because he fathered five children who love him so much, and Lord knows, we sure miss him. We have great faith that we will be reunited, and he told me before he passed that he wanted me to LIVE BIG! He said do not worry about other people because people-pleasing will run you down, sweetheart. 💓Shannon Knight – I thought of my dad all day today., it would have been his Birthday…
He said, fulfill your dreams right now because we are not promised one day. He said so many things that made my faith grow stronger while he was battling cancer. All dad cared about in his final months was his children knowing how much we meant, how he loved us and how God gave each of us a soul purpose.
Our souls will long, long outlive these bodies we are in for a very short time.He taught us so many things and he was laser-beam focused and determined for us to fully understand our existence. Soul first…spirit, heart and a very temporary body. He told us how he appreciated our talents and individual gifts from God. He said to keep my circle tight, let loving people in, and keep the bullies out. I did precisely that and kept my promise.

My life changed dramatically since he stepped into heaven. I heeded every bit of advice he gave me. I recorded our conversations, and wow, are they filled with great words of wisdom. Dad made sure to let every last breath count in the conversations with his children. I am in awe of how all he cared about was staying close to Christ in prayer during his last months. He talked about how excited he was to reunite with his family, grandma and grandpa, uncles, aunts, and most importantly, meet Jesus!
I never expected such an exemplary example of faith:

We battled cancer together and took different paths to heal, and we always agreed that God, our creator, was in every clinic and hospital regardless of the road we chose to try and get well. God is the One who knows; His will and purpose for us are much more than we can imagine. He will find a purpose for us even when we are sick, and we should be aware- even if it is praying for others who prayed for us. We all have a purpose at every stage of life, and every day we have choices of what we can do with our life.

When he stepped into heaven on May 31st, 2019, at 11:55 AM, the song was playing on the intercom where I was getting my hair done. It was pretty incredible. 😇On July 19, 2006, I was diagnosed with breast cancer. It is my youngest brother’s birthday; I’ll never forget it. I went through a rollercoaster ride trying to heal which lasted four and a half years. My own cancer journey caused anxiety and fear which is normal and it didn’t mean that I lacked faith in God. I was facing fear at the same time as holding onto faith.
In July, 2010, when I had a shocking recurrence and developed metastasis in my lungs, sternum, ribs, and clavicle, my faith in the midst of fears was tested. I did 22 of the recommended radiation treatments to my sternum but could not complete the radiation due to staph infection and unfortunately the cancer had only progressed.  I remember praying during my treatments. When my radiation oncologist said I was too sick with complications to complete it my fear only increased. I couldn’t do chemotherapy because it had too many risks.
September 9, 2010, UCLA gave me only months to live. I never gave up, though. We raised money and I took a different path, then conventional cancer treatment, and went to CMN Hospital in San Luis, Mexico. www.cmnact.com

I’m glad I prayed and learned to lean more on God. Fear is a natural response when you hear that your life is ending soon.
I prayed for help and held on to hope. I had to put my faith first, trust my intuition, and not let anything or anybody get in the way of choices.I believed it was right for me. Not everyone agreed with my choices, but here I am, alive and healthy. No one is ever prepared to receive a cancer diagnosis. I understand the questions that go through our heads, such as, “What did I do wrong?” or “Why me?” I have learned that cancer does not discriminate and that no one is ever prepared for a cancer diagnosis.Every treatment choice I made at each crossroad felt like an enormous responsibility to me. As someone who has made those decisions for 4-5 years without knowing the outcome, I respect the choice of each individual. Never regret the treatments you chose to heal your body or that did not work for you.

There are people who DO LOVE YOU, who do understand what love is …love them. 🥰🥰Letting go of someone even if it’s a family member, or friend is NOT an act of anger, it is not abandoning when someone does not want what love you have to give. It is surrendering …with love – where you can still love them, still pray for their happiness while letting go. It may be crazy to think that a family member may not love another family member, but you’d be ignoring history to think it impossible.
My father taught me to surround myself with people who love you because you deserve it and that lesson from him saved my life back in 2010,  You can love everybody as a brother or sister and that is beautiful but to expect people to reciprocate how we feel about them is not reality. You can feel sadness. You can feel frustration but do come to terms with the reality of how love is expressed is easy to recognize.
I speak with women almost every day and more than once a day and the reality is that many of them are sad because they don’t get the love they want from someone they love, and they could be wasting their time going in a direction that is not meant for them. It is not their path. When we let go, we make room for other blessings meant for us🙏🤗✝️🥰.

The Truth About Hydrogen Fuel Cell Cars    

Are Hydrogen Cars a Type of Electric Vehicle (EV)?

One type of vehicle that’s not talked about all that much is hydrogen cars, which are a work-in-progress type of technology. While hydrogen fuel cell vehicles offer great benefits, there are still too many limitations, such as a lack of infrastructure across the country, which is why there are only about 15,000 of these units on the road now. Whether they’ll be big competitors to electric vehicles will remain to be seen, at least for now. 

One question that often enters people’s minds is whether a hydrogen car is another type of electric vehicle or if it’s in a classification of its own. Let’s explore what a hydrogen-fueled car is, whether it’s an EV or not, and whether any of these vehicles are currently on the U.S. market.

Hydrogen is a little-known alternative fuel source
Electric vehicles are meant to help keep the environment clean of hazardous emissions and transition to a more renewable and cheaper fuel source. Hydrogen cars, which have
a different technology, emit only water vapor from the car’s exhaust pipe, so it’s also considered a zero-emission vehicle, according to Car and Driver.
Essentially, it’s a specific type of EV, its classification is the same as an electric vehicle.
Powering up this kind of car is the same motor found on an EV, except it uses a fuel-cell unit instead of the typical battery you find with an electric vehicle. The fuel cell has pure hydrogen running through a membrane inside the unit, which will combine with oxygen from the air, producing energy to move the car forward.

To fill up the tank in the vehicle, you will have to use a hydrogen fueling station, which generates pure hydrogen by breaking up compounds containing the molecule, such as natural gas. However, because the process is complicated and expensive, the cost of fueling up at a station will put quite a dent in your wallet. The price could range from
$10 to $17 per kilogram, equivalent to approximately $5 to $8.50 a gallon of gas.
Another constraint on a Hydrogen fuel cell vehicle is that there are relatively few stations you can fill up at. Currently, the 60 or so fuel stops are only found in California, which is why you can’t buy this car in any other state. Honda used to offer the Clarity Fuel Cell model for a while, but due to the limitation of fuel stations, among other reasons, the brand discontinued it.

What are the 4 types of EVs available?
One of the most common types of EVs are ones with hybrid technology.
This combines an electric motor with a gasoline-powered engine. The two units work together to increase driving ranges and decrease vehicle emissions. The battery that runs the electric motor can’t be charged with an outside source; it recharges from the energy recaptured from the car. Offering impressive fuel economy ratings, an excellent example
of this hybrid type of EV is the Toyota Prius.

The next type is a plug-in hybrid vehicle (PHEV), which runs much like a regular hybrid, except you can charge the electric motor using an outside charging source and recharge from the energy produced from the gas engine. Also, according to the U.S. Department of Transportation, you can travel anywhere from 20 to 50 miles on electricity, and then the internal combustion engine can take over. An example of a popular PHEV is the Toyota RAV4 Prime.

The most well-known type of EV is the battery-electric model.
This refers to the fully electric vehicles we see on the market today. The battery stores energy and transmits it to the motor to move the car down the road. It can be recharged using an external power source, accessed from your home, or a charging station in public. All Tesla models and popular vehicles like the Chevy Bolt and Nissan Leaf fit this category.

The last type is the fuel cell vehicle, where hydrogen technology comes in.
To power your car, the vehicle will convert hydrogen into electricity, providing the motor with energy to move the car in the direction you want to go. To fill up the tank, you must use a special hydrogen station.

Are there any hydrogen-powered vehicles currently for sale in America?
So far, only two vehicles on the market use hydrogen as a fuel source. One is the Toyota Mirai, and the other is the Hyundai Nexo. Both of these cars are currently only for sale in California because the only places you can fuel up are in that state. 

The Hyundai Nexo’s fuel cell motor offers 161 horsepower and 291 lb-ft of torque, which isn’t all that powerful, considering you only get about an 8.4-second acceleration time on a 0 to 60 mph run. As for fuel economy, you can expect to get approximately 61 MPGe and a driving range of around 380 miles for the standard-sized tires. Opting for the 19-inch wheels decreases the range by 26 miles.

of torque. On a 0 to 60 mph run, this model will take 9.1 seconds to get up to speed,

Among the various technologies to which the future of the automobile is turning, the hydrogen fuel cell car is already a reality. This technology was introduced on a vehicle from the Renault utility range — Kangoo Z.E. Hydrogen — for the first time in 2014. Hydrogen is changing the game in electric mobility, both in terms of challenges and possibilities.

Here are some explanations to better understand this famous chemical element.
While, for many, the term “fuel cell” might sound like complex advanced technology,
the secret to this new way of producing energy also lies in a simple chemical reaction
between oxygen and hydrogen. So what are the particularities of a hydrogen vehicle?
How does it work? What are its advantages? And, more concretely, also what are the
possible uses of a hydrogen car in everyday life?

The challenges of the hydrogen electric car
We’ll begin with some context. The term “hydrogen electric car” refers to a vehicle powered by a specific energy source — hydrogen — using a special device: the fuel cell.
The hydrogen fuel cell vehicle belongs to the large family of electric cars, as it gets its traction from an electric powertrain. For an electric car powered by a lithium-ion battery, electrical energy is simply stored in the battery after charging on the electrical grid.

Herein lies the difference with the hydrogen vehicle. The electricity required to run the powertrain is not only supplied by the battery, but also, by the fuel cell using hydrogen stored onboard the vehicle. The majority of hydrogen vehicles currently produced by automobile manufacturers work in this way.

The hydrogen-powered car shares the same emission-free driving ambitions as the “conventional” electric vehicle. That said, there still remain several challenges to overcome in order to reduce the environmental impact of hydrogen production as much as possible. The current method of extracting this chemical element relies on the steam reforming of hydrocarbons and is called “grey” hydrogen because it is extracted from fossil fuels.

But there is another way to obtain hydrogen:
Through water electrolysis. Where the electricity used in the electrolysis process
is from a sustainable energy source, like solar or wind power, it is possible to produce
“green” hydrogen. This extraction method is a solution for the future.

How does the hydrogen fuel cell vehicle work in practice?
Its electric energy is supplied by a fuel cell. Pressurized hydrogen is stored in special tanks onboard the vehicle. The gas (H2), along with dioxygen (O2) from the surrounding air, are supplied to the fuel cell. These two gases then undergo an electrochemical reaction inside the cell, in turn producing electricity, heat and water vapor (H2O), which is released in the form of a gas via a small tube located underneath the vehicle.

The resulting energy produced:
As well as energy from the battery, power the car’s electric motor, which is then able
to run silently and with zero pollutant or CO2emissions*. When it comes to replenishing
the hydrogen, fueling takes place at dedicated stations using pumps that inject hydrogen,
extremely quickly, into the vehicle’s tank in the form of pressurized gas.

How much does a hydrogen fuel cell electric vehicle consume?
Bear in mind that the hydrogen atom, two of which are required to make up the dihydrogen (H2) molecule, is one of the simplest and lightest natural elements on the periodic table (a classification which lists all the chemical elements present on Earth.)
It has very low volume density. So, in order to obtain the quantity of hydrogen needed
to power a vehicle, it is necessary to store a considerable amount in large tanks at high pressure.

In this operational diagram of the Kangoo Z.E. Hydrogen, the motor of the hydrogen-powered electric car is powered by its lithium-ion battery. As for the fuel cell, it provides an additional energy reserve that increases the range of the car (providing more autonomy and shorter charging time.) Hydrogen-powered vehicles provide the driver with practical and financial advantages of electric power, plus the benefits offered by hydrogen in terms of flexibility and range.

The advantages of the hydrogen-powered car.
Behind the wheel of a hydrogen-powered vehicle, the driver benefits above all from almost double the range. As part of the complementary relationship between the fuel cell and the lithium-ion battery, the electricity produced by the fuel cell using the stored hydrogen is added to the storage capacity of its main battery.

For example, Renault’s Kangoo Z.E. Hydrogen is equipped with a 30 kWh
battery coupled with an onboard hydrogen storage capacity equivalent to 29.7 kWh.
Its range therefore increases from 230 kilometers to 370 kilometers (WLTP* cycle).
In this way, the hydrogen system can be seen as a “range extender”.

The icing on the cake is the refueling speed.
As well as recharging on the grid, a maximum of ten minutes is enough to refill the hydrogen tank in order to supply the fuel cell — instantly increasing the vehicle’s range. And these aren’t the only advantages: the hydrogen-powered vehicle offers all the benefits of driving an electric vehicle, starting with the lack of engine noise, enjoyable driving experience, and access to restricted driving zones in certain city centers.

How exactly is a hydrogen fuel cell electric vehicle recharged?
The answer differs between manufacturers.
At Renault, there are two ways of restoring the vehicle’s energy reserves.
Firstly, recharging from a standard electric terminal to supply the lithium-ion
battery (an option facilitated by the number of accessible charging stations.)
Secondly, refueling at a hydrogen station to refill the gas tank takes just a few
minutes during the day, for example, when the driver needs to extend their trip.

Hydrogen storage.
While the term “fuel cell car” has not yet entered into our everyday language, it shouldn’t evoke concern when it comes to safety. Of course, hydrogen, highly-pressurized in its tank, is a volatile and flammable element. In case the fuel cell overheats or is removed, the hydrogen is dispersed and released in less than a minute. Before going into circulation, hydrogen cars must adhere to particularly strict European regulations.

Kangoo Z.E. Hydrogen is subject to a European certification in accordance with
the EC regulation n°79/2009. For over twenty years, technologies that use dihydrogen have been tested and approved on numerous occasions, often under extreme conditions (space engineering, submarines, construction machinery). Today, thanks to the experience accumulated from the pioneering Kangoo Z.E.

Hydrogen since 2014, Renault Group is going even further:
Announcing a partnership with the renowned world leader in fuel cells,
Plug Power, who have rolled out over 40,000 fuel cell systems to date. The resulting Innovation Center will not only place its partners at the cutting edge of research and development, manufacturing, and the commercialization of hydrogen vehicles, at the forefront of related services (charging infrastructures, hydrogen supply, etc.)

While these two technologies give the impression of being diametrically opposed,
they are actually complementary mobility modes.
The question “hydrogen or electric car?” depends above all on the use and type of vehicle.
On a light utility vehicle with a high payload capacity, hydrogen is particularly interesting.
Even though the tank used to store the gas is large, its weight remains reasonable.
Hydrogen is especially suited to intensive closed loop use where refueling is easily
locatable and accessible on the vehicle’s route, or even directly on company premises.

Electric vehicles equipped only with lithium-ion batteries are intended
for all types of driver and use, from short trips around town to longer journeys.
But when the battery is empty, the only solution is to recharge on the electricity grid.
The electric-only vehicle takes longer to charge than its hydrogen counterpart, but
there are more charging stations available, both in private and public locations.
Parking can be easily combined with charging the vehicle. Both electric and hydrogen
are complementary technologies designed to provide sustainable mobility suitable
for all types of use.

How established are hydrogen vehicles in Europe?
In 2020, new registrations of hydrogen fuel cell vehicles occurred mostly in Germany, followed by the Netherlands and France**. Hydrogen refueling stations are expanding progressively throughout Europe. In 2020, for example, there are around 100 stations in France, with the objective of reaching 1000 stations by 2030. There are 150 operational stations in Europe, with 750 being rolled out. 3,700 stations are in the works for 2025.

To support this developing infrastructure, several European countries have introduced incentivizing policies to promote the technology: like Germany, which has just allocated
a budget of 700 million euros to the sector, and France, which finances the purchase of a hydrogen vehicle by up to 16,000 euros. The arrival of the first mass-produced hydrogen electric cars, including professional vehicles currently refueled on company premises, will help accelerate the development of infrastructure, promising a bright future for hydrogen technology.

You might not think much about hydrogen-engines, but they’re actually similar to
gas-powered motors in a lot of ways. They both use a four-stroke design for intake,
compression, ignition, and exhaust, and both make the same sorts of sound.
But, as Jason Fenske of Engineering shows us, there are some pretty big differences.

The main difference is what sort of exhaust hydrogen engines put out. Instead of toxic NOx gases, hydrogen motors produce water (yes, plain, old water) as the main bi-product of their combustion cycle. Because of the heat also produced by the engine, there are still some harmful NOx emissions, but not nearly as much as a normal gas engine produces.

Because hydrogen is different from gas, the sorts of air-fuel ratios, compression ratios, timing and ignition energies you can run are wildly different from what you’d normally see. For example, you can run an air-fuel ratio as lean as 180:1, and a much higher compression ratio since hydrogen has a higher-octane rating. It’s all pretty technical,

Gas prices declined last fall after oil supplies rebounded and demand cooled.

What’s Happening with Gas Prices?
But prices have been rebounding early in 2023, leaving analysts and consumers
anxious over what could happen next for prices at the pump. The cost of crude oil,
which plays a big role in what consumers pay at the gas pump, that slid during the
second half of 2022 as oil traders anticipated weaker demand & a global economic
downturn.
Another reason prices dropped was a boost in fuel supplies after President Joe Biden
set in motion a record release of oil from global petroleum reserves. In July, the Treasury Department estimated that the releases, both domestically and internationally, reduced gasoline prices by as much as 40 cents per gallon. But oil prices are back on the rise as global demand for crude rebounds, driven by China’s emergence from Covid lockdowns.

There’s concern that oil supplies could be squeezed further by a worsening of the
Russia-Ukraine war. Meanwhile, the U.S. is taking some oil off the market—by refilling
its strategic petroleum reserve. “Concerns are increasing that without additional oil,
supply will tighten in the weeks ahead, especially as the nation starts to move away
from softer demand in the height of winter,” says Patrick De Haan, head of petroleum
analysis for GasBuddy.
“Moving forward, it doesn’t look good for motorists, with prices likely to continue accelerating,” De Haan says. GasBuddy has predicted pump prices could hit a national average of $4 before and during the summer driving season, with prices of around $7 possible in California’s largest metro areas. Gas prices may go higher than they were a

These apps require little commitment compared to loyalty and rewards programs

Circle K Easy Pay
Another way to save at Shell is with Circle K Easy Pay Debit.
This program allows you to save 30 cents per gallon for the first 100 gallons
or 60 days, depending on your location, after which you’ll save 10 cents per gallon.
Here’s how it works: You get a physical Easy Pay card that’s linked to your checking account. You then use that card to pay for your fill-ups, with all savings automatically deducted. Cards are PIN-protected and become locked after three failed login attempts. The only downside is that you can’t earn credit card rewards this way.

Pilot-Flying J myRewards Plus
Pilot and Flying J have a joint loyalty program called myRewards Plus which lets drivers earn rewards on fill-ups and other purchases. Through Oct. 31, you can get a discount of
10 cents per gallon on all fill-ups when you pay for gas with the myRewards Plus app.
This promotion should work at all Pilot and Flying J locations around the country.

Upside
This third-party app hit the scene a couple of years ago and has expanded significantly.
It now includes restaurant and grocery store rewards, as well as savings at the pump.
You can claim offers in the app to save a certain number of cents per gallon.
After you claim an offer, you have four hours to fill up at the specific station you selected, using a linked credit card. Cash back earned from the fill-up will then be placed in your Upside account, which you can cash out whenever you like.
I like Upside because fill-ups with savings are good for up to 50 gallons.

Pay with GasBuddy
The Pay with GasBuddy program continues to evolve rapidly and improve.
It used to offer Shop Your Way points, but now it offers several levels of cash
savings per fill-up. It introduced deals you can claim at specific gas stations
right before you fill up, as well as a “gas back” shopping portal in the app.

Similar to Circle K Easy Pay, payment occurs with a physical card GasBuddy
mails to you that is connected to your checking account. You pay with it at the pump.

GasBuddy also offers a Pay with GasBuddy Premium option where you can save up
to 40 cents per gallon after enrolling. This service costs $9.99 per month and guarantees
that you save at least 20 cents per gallon (on up to 50 gallons per month).

It also offers additional perks like roadside assistance.  
According to a GasBuddy Study, you will save $250 a year by filling your tank on Monday compared to other days of the week. Why? Demand for gas increases on Thursday based on weekend travel. By Monday when demand drops stations have to lower the cost to bring customers into the stores. Sunday’s are the second cheapest day of the week for fuel. 

Wish you can predict when the cost of gas will go up or down. You can!
All it takes is finding out the current price of crude oil, which is the main ingredient in gas. At  Crude oil – 2023 Data – 1983-2022 Historical – 2024 Forecast – Price – Quote – Chart (tradingeconomics.com) If the price goes up. you will have three days to purchase gas before prices at the pump rise. If the price drops, you can see the price drop in about three days. The reason?

There’s usually a lag period how quickly station owners respond to the fixed price. 
Also gas prices are usually lower in locations where other gas stations are close by.
That’s because competition keeps prices in check.
According to a study from the University of California, Santa Barbara fuel is priced
lower at convenience stores that sell coffee, subs and foods because the amenities
subsidize the cost of the gas, allowing owners to charge less for gas.

Grocery store fuel points

Nowadays, almost every nationwide grocery store has its own fuel points program
or has partnered with an existing national gas station brand’s program to offer fuel
savings for your grocery shopping. The newest entrant is Walmart +, Which is ….
Walmart’s subscription service offering free two-day shipping on Walmart.com
orders and other perks, similar to Amazon Prime.

There are several other grocery chains offering gas discounts for being a member.
Kroger is a very popular option in the South. Between regular family grocery shopping
and taking advantage of 4 fuel points per dollar on gift cards, it’s pretty easy to max out
the $1-per-gallon savings the program allows. You can even buy third-party gift cards
from the Kroger gift card-specific website. It regularly has gift cards for sale at discounts
or with earnings of 4 fuel points per dollar. 

Stack programs
You can stack two or more of the above programs on a single fill-up
to compound savings and make gas particularly cheap. Here are a few strategies:
Fuel Rewards plus Pay with GasBuddy: The two programs operate independently of each other, allowing you to lower the price at the pump with Fuel Rewards. You’ll then have less cash than you pump deducted from your checking account when paying at a Shell station with your GasBuddy card. If you earn enough Fuel Rewards to make fill-up basically free, GasBuddy still gives you the savings based on the pre-discounted price of the Shell gas — meaning you can make a profit filling up your tank with gas.
Kroger plus Pay with GasBuddy: The same principle as above applies when filling up
for a discount using Kroger Fuel Points and then paying with your GasBuddy card.

Fuel Rewards plus Circle K Easy Pay: 
Like the two strategies discussed above,
you can earn Fuel Rewards when paying with your Circle K card.
Stacking with Amex Offers: From time to time, American Express offers promotions on gas purchases. For instance, we’ve seen Amex offer 400 bonus Membership Rewards points when spending $40 on gas. These stack with gas station and third-party loyalty programs, though you must pay with the Amex card you linked the offer to. As such,
you can’t stack Amex Offers with Pay with GasBuddy or Circle K Easy Pay.

With the average price of gas over $4 for the first time in fifteen years, you might be struggling to afford to fill your tank up. But luckily, learning a few tips and tricks can help save you money on gas. Here are some easy ways to slash your fuel costs and boost your bottom line.

Get more bang for your buck
First, track the cheapest gas prices through a free app, such as Gas Guru.
“And bring cash — at many stations, you’ll save 10 to 15 cents per gallon,” says budgeting expert Andrea Woroch. If you are in the market for a credit card with rewards, look at your end-of-year credit statement to see which category you spend most on overall — it may not be gas, says Woroch. “You may end up earning more points with a card that gives you flat-rate rewards, like 2 percent on everything you buy. It depends on your spending habits.” Go to CardRates.com to compare rewards. 

Avoid pricier days
You may have heard prices at the pump surge on Fridays, as folks hit the road on the weekend. That used to be true, but trends are shifting, says GasBuddy spokesperson Nicole Petersen. “Our annual study for 2022 shows that the cheapest day of the week is still Monday, but the most expensive days are now Wednesday and Thursday,” she reveals, surmising that over the pandemic, our habits have changed, with more of us making midweek plans to travel or take short trips. “This is the first year we’ve seen such a shift, but it remains true that the best time to fill up is early in the week.” 

Stack savings
“Fuel rewards programs — at participating stations like Shell, Exxon,
and BP — are your secret weapon,” says Petersen. And she does mean secret!
“Our survey found that 46 percent of people don’t belong to a loyalty program —
it’s an underutilized tool because it saves drivers $50 to $100 per year,” Petersen notes.
“Sign up for a loyalty program at, say, Kroger or Stop & Shop, and for each dollar you spend on groceries, you’ll earn one fuel point at participating stations,” adds Woroch.
“For every 100 points you rack up, you’ll earn ten cents per gallon.” Don’t stop there:
Download a cash-back app, like Fetch Rewards. “They increased fuel rewards from five
to 20 points per receipt — 1,000 points earns you a gift card,” Woroch says. 

Drive smart and ‘bundle’ trips
Taking several short trips consumes almost twice as much gas as “bundling” several errands in one outing. That’s because the engine burns through more gas from a cold
start and is more fuel efficient when the car is already warm, says AAA spokesperson Andrew Gross.
And before you head out, lighten your load: “At highway speeds, 50 percent of engine power goes to decreasing aerodynamic drag, so removing gear from your trunk will boost fuel efficiency,” Gross adds. Indeed, ditching “dead weight” can save you three cents per gallon — and every bit counts! 

Be highway savvy
You’ve heard it before, but it bears repeating: Go easy on the pedal.
“Speeding lowers fuel economy by 15 to 30 percent,” says Petersen.
Indeed, for every five mph we drive above 50 mph, we waste 22 cents per gallon.
Also smart: Turn on your A/C if you’re going over 50 mph. “At highway speeds,
rolling down windows decreases fuel efficiency by 20 percent,” Petersen explains.

Skip ‘tanker time’
If you pull into the station and spot a tanker truck supplying the gas terminals,

Low Vitamin D Levels

It’s well established that having a low level of vitamin D can lead to bone disease,
like rickets in children and osteoporosis in adults. But it may also increase the risks
for heart attack and premature death.
The Endocrine Society defines vitamin D deficiency as a blood level of less than 20 nanograms per milliliter (ng/mL), while vitamin D insufficiency is 21 to 29 ng/mL. Optimal levels are 30 to 100 ng/mL.

In a study published in October 2021 in The Journal of the Endocrine Society,
researchers identified 19,092 people who showed vitamin D deficiency on at least
two tests and who’d never had a heart attack. Everyone was divided into three groups. People in group A were not treated, and their blood levels remained below 20 ng/mL. Those in group B received vitamin D supplements that raised their levels into the range
of 21 to 29 ng/mL. Those in group C took enough supplements to raise their levels to
30 ng/mL or higher.
The researchers found the risk of heart attack was lower in group C than in both of the other groups. However, people in groups B and C (who all took supplements) had a lower risk of death from any cause than those in group A, who remained deficient in vitamin D. The connection? Other evidence has linked low blood levels of vitamin D to cardiovascular risk factors, such as high blood pressure, diabetes, obesity, and chronic kidney disease.

Research study into vitamin D and calcium
Researchers based at the Yonsei University College of Medicine in Seoul, Republic of Korea, set out to evaluate the link between vitamin D and calcium and the number of patients needing revision surgery after total knee replacement surgery. Using the Korean National Health Insurance database, researchers looked at patients diagnosed with knee osteoarthritis who had primary knee replacement surgery between 2009 and 2018.
They ranked the number of patients at risk of needing revision surgery using a model
called the Cox proportional hazards model.

Of the 142,147 patients assessed, 28,403 used vitamin D and calcium while 113,744 didn’t. The study found that calcium and vitamin D significantly reduced the risk of revision knee replacement surgery. Both patients with periprosthetic joint infection and those without infections had less chance of needing revision surgery if they took calcium and vitamin D for more than a year after their knee replacement surgery. 
Similarly, implant survival rates were higher in people who took supplements for more than a year compared to those who did not. In patients who developed an infection after surgery, vitamin D and calcium taken for more than a year increased the five-year implant survival rate from 75% to 93%. In patients without infection, researchers noted a reduction in implant failure rates of 58%. The researchers concluded that a dose of 800 IU or higher of calcium and vitamin D for more than a year after knee replacement surgery lowers the chance of needing revision surgery.

1. Introduction 

Vitamin D plays an important role in the musculoskeletal system of the human body. Traditionally, much of the focus on vitamin D has been placed on the regulation of bone health and fracture healing. However, recent growing evidence has shed light on the importance of vitamin D and its relation to soft tissue healing and function.
Despite recent research, there continues to remain confusion about the role of vitamin D.
Here, we review the most current literature on vitamin D as it relates to orthopaedic surgery and the musculoskeletal system, focusing largely on non-fracture applications.
We outline the basic science of vitamin D metabolismepidemiology of vitamin D levels, roles vitamin D serves within the musculoskeletal system, and the correlation of vitamin D with injuries and surgical outcomes.

2. Background

2.1. Sources of vitamin D
Vitamin D is biologically synthesized using ultraviolet B (UVB) rays from the sun.
Solar UVB rays transform 7-dehydrocholesterol to provitamin D3 at the skin level (Fig. 1). Pre-vitamin D3 is also acquired in small quantities through the consumption of liver, cod liver oil, fatty fish, and egg yolks [1][2][3]. From the skin, pre-vitamin D3 is attached to vitamin D binding protein and transported to the liver where it is converted to 25-hydroxyvitamin D3 (25 OHD), also known as calcifediol [1][3].
Through an additional hydroxylation step, predominantly in the kidneys, 25 OHD is
converted to 1,25-dihydroxyvitamin D3 or calcitriol, the active form of vitamin D.  [1][3][4].

2.2. Basic mechanisms
Vitamin D is a secosteroid with direct hormonal interactions with over 200
genes in the human genome [5]Vitamin D receptors (VDR) on target cell nuclear membranes facilitate the endocrine and autocrine functions of vitamin D in multiple tissues [1] The endocrine function of vitamin D is largely present in the small intestine where it is instrumental in regulating serum calcium and phosphate absorption.
This modulation of serum calcium and phosphate is intertwined with the actions of parathyroid hormone (PTH) and, consequently, bone mineralization [1][3].
 
Additionally, it stimulates osteoblasts causing osteoclast activation, a crucial step in calcium mobilization and bone remodeling [6]. In addition to the endocrine effects
of vitamin D, it plays many important roles in muscular development and function.
This includes regulating phosphate accumulation in myocytes, aiding muscle function,
and metabolism after conversion to creatine phosphate.
It is also theorized to play a direct role in protein synthesis and growth.3 
Vitamin D has been found to alter myocyte responsiveness to insulin,
indirectly improving muscle recovery and construction. 

Lastly, it has been theorized that type II muscle fibers have a greater amount of VDRs
than type I, leading to type II proliferation when a hypovitaminosis D individual increases their serum vitamin D concentration [7]. Cartilage and joint capsular tissues have also been evaluated for their relationship to vitamin D.
One theory is that the vitamin D metabolite 24R,25-dihydroxyvitamin D3 plays a role
in stimulating extracellular matrix proliferation by chondrocytes. Through these effects, vitamin D plays an important role in optimizing mechanical loading [8].

2.3. Vitamin D levels
There is considerable debate as to the level of serum 25 OHD that designates a person as vitamin D deficient, insufficient, or sufficient, and there has been no minimal clinically important value established for vitamin D. The Vitamin D Council reports deficient as
0–30 ng/mL, insufficient as 31–39 ng/mL, and sufficient as 40–80 ng/mL [1].
The Endocrine Society reports deficient as 0–20 ng/mL, insufficient as 21–29 ng/mL,
and sufficient as 30–100 ng/mL [9]. The Food and Nutrition Board Institute of Medicine reports deficient as 0–11 ng/mL, insufficient as 12–20 ng/mL, and sufficient as greater than 20 ng/mL [10][11]. Thus, further studies are needed to help standardize normal ranges for vitamin D. In addition, levels categorized as deficient or insufficient should be clinically important and correlate with
injury and healing variables.

2.4. Vitamin D supplementation
Vitamin D supplementation is available in the form of vitamin D3 (cholecalciferol),
or vitamin D2 (ergocalciferol). Although there is some debate, vitamin D3 appears to
be preferable for supplementation, with varying ranges of recommended dosages  [9][10][11][12][13][14] 
According to the Institute of Medicine, the recommended daily allowance (RDAs) for vitamin D is 600 IU for ages 1–70, and 800 IU for ages 71 and older, while the tolerable upper level (UL) is 4000 IU per day for ages 9 and older [10][11]. Treatment dosages
for low vitamin D levels are varied, with ranges between 400–1000 IU per day [13]. Recommended dosages for patients with 25OHD levels between 25–30 ng/mL are
2000–4000 IU D3 daily, and 50,000 IU once a week of D2 for 8 to 12 weeks
for those with serum 25OHD levels < 25 ng/mL [9][13]

3. Epidemiology
The National Center for Health Statistics estimates up to 32% of Americans
are deficient in vitamin D, and up to 1 billion people worldwide are estimated
to be insufficient or deficient [1] 
Prevalence of vitamin D deficiency is likely to increase due to time spent indoors and
increased use of sunscreen for skin cancer prevention. Risk factors for hypovitaminosis D
 include latitudinal location, winter season, skin melanin abundance, clothing coverage,
 kidney disease, and Crohn’s disease [1][3][15].

3.1. Athletes
Recently, there have been numerous studies evaluating the prevalence of abnormal vitamin D concentrations in the athletic population at both the collegiate and professional levels. In 2012, Villacis et al. found that 33.6% division I athletes at the University of Southern California had abnormal vitamin D levels [16].
Specifically, males, African-Americans, Hispanics, and those with a darker skin complexion were at increased risk for having low vitamin D levels. Barcal et al. showed that 74% of collegiate wrestlers had levels < 32 ng/mL in the fall, which increased to 94% in the winter and spring seasons [17]
They also found an association between low vitamin D levels and higher adiposity.
The changes in vitamin D levels on a seasonal basis are well documented. Vitale et al. performed a prospective cohort study analyzing vitamin D levels in 152 professional alpine skiers over the course of 3 full years in order to evaluate fluctuations in vitamin D [18]. Fifty one percent of these athletes at one point were vitamin D insufficient and 30% were deficient at one point over the course of three seasons.

Most athletes’ vitamin D levels peak in the summer and are lowest in the winter.
At the professional level in the United States, the prevalence of low vitamin D levels
has been studied in the National Basketball Association (NBA), National Football League (NFL) and National Hockey League (NHL). In a study by Fishman et al., 90 (32.3%) of the 279 NBA players at the NBA combine from 2009–2013 were vitamin D deficient, while 131 (47.0%) were vitamin D insufficient [19]. The mean vitamin D level in this cohort was 25.6 ± 10.2 ng/mL. Similarly, Angeline et al. found that 30% of NFL players on a single NFL team were vitamin D deficient (< 20 ng/mL), while 51% were vitamin D insufficient (20 to 31 ng/mL) [20]. In this cohort, lower vitamin D levels were associated with muscle injury. 
Maroon et al. found that African-American NFL players were more likely than white players to be vitamin D deficient [21]. In contrast, Mehran et al. found a low prevalence of vitamin D insufficiency (20–31.9 ng/mL) in the NHL, with a rate of only 13.3% in 105 hockey players across 3 NHL teams [22]. However, there were major differences in racial composition in these cohorts, with a higher percentage of white athletes in the NHL cohort compared to the NFL study. 3.8% of athletes were black and 96.2% of athletes were white in the NHL study cohort, while 84% of athletes were black in the study by Maroon et al. [21][22] Some authors espouse a 25-OH vitamin D level > 40 ng/mL as ideal in athletes.

3.2. Orthopaedic patients
Many studies have evaluated the deficiency of vitamin D in patients undergoing orthopaedic procedures. Observational cohort studies in the United States and internationally have demonstrated a high prevalence of hypovitaminosis D in these patients [23][24][25]. Additionally, recent studies have demonstrated high rates of vitamin D insufficiency and deficiency in pediatric orthopaedic patients and
adult patients undergoing foot and ankle, spine, joint replacement, and orthopaedic trauma surgeries [26][6][27][28][29]. Inkrott et al. retrospectively reviewed vitamin D levels in 218 patients undergoing shoulder arthroplasty and found that 43% of patients were vitamin D insufficient (< 30 ng/mL) and 11% were vitamin D deficient (< 20 ng/mL) [29]. In this study, low vitamin D levels were associated with increased BMI (≥ 30 kg/m2), similar to the study by Barcal et al. [21].

4. Musculoskeletal systems

4.1. Intra-articular cartilage
Recent studies have specifically investigated the effects of vitamin D on cartilage health. A systematic review by Garfinkel et al. about vitamin D and cartilage found that patients with decreased articular cartilage thickness were more likely to be vitamin D insufficient and concluded that low levels of vitamin D is a risk factor for the development of osteoarthritis (OA) [30]. Using a rodent model, Pascual-Garrido et al. found that low vitamin D levels had deleterious effects on articular cartilage as determined by histological analysis and micro-CT [31].
 Castillo et al. conducted a rat study investigating the relationship between vitamin D and the biochemical mechanisms of cartilage loss, inflammation, and OA progression and found that vitamin D supplementation had a protective effect during the initial stages of OA but not during the chronic stage [32]. In another rat study by Li et al. using ovariectomized rats, the authors found that vitamin D prevents cartilage loss through the regulation of type II collagen turnover [33]
Rai et al. evaluated the role of vitamin D in inflammation, fatty infiltration, and cartilage loss in the knee in micro swine fed a high-cholesterol diet and found increased inflammation in knee joint tissues, increased fatty infiltration in the soft tissue, and increased chondrocyte clustering in the articular cartilage, a hallmark of OA, in the vitamin D deficient and sufficient groups compared to the group supplemented with vitamin D [34].

4.2. Muscle function
Tomlinson et al. conducted a meta-analysis assessing the effects of vitamin D supplementation on upper and lower body muscle strength [35]. Results showed that there was a significant increase in muscle strength when healthy adults ages 18–40 received vitamin D3 supplementation. Rebolledo et al. performed a retrospective cohort study on the 214 athletes at the 2015 NFL combine to assess vitamin D levels and lower extremity and core muscle strains [36]. Fifty percent of athletes reported a lower extremity muscle strain or core muscle injury.
 Athletes who reported a history of injury had significantly lower levels of vitamin D compared to those who reported no injuries (p = 0.03). Additionally, athletes with inadequate levels of vitamin D had 3.61 higher odds (p < 0.001) of a hamstring strain compared to those with normal levels. However, the study was retrospective and the authors did not account for seasonal changes of vitamin D levels. Dawson-Hughes reviewed the relationship between vitamin D and muscle function and found that older adults with 25 OH D < 40 nmol/L were most likely to improve their muscle performance on strength testing with supplementation (dosing of 800–1000 IU/day) [8]. Conversely, in a randomized control trial by Saha et al. assessing the effect of calcium and vitamin D supplementation on testosterone levels and muscle strength in young males (age 20.2 ± 2.2), there was no statistically significant evidence suggesting a positive relationship with supplementation, testosterone, and muscle strength after 6 months [37].

4.3. Falls in the elderly
In the elderly, one notable area of investigation is the relationship between vitamin D and physical performance, including falls, a major cause of morbidity and mortality in this population. In reviewing the literature on the effect on vitamin D on falls and physical performance, Dhaliwal et al. found that there may be an association in older adults, although the existing literature is inconsistent [38]. However, excessive vitamin D supplementation and high serum 25 OH D levels can also increase the risk of falls in the elderly. Smith et al. conducted a double blind randomized clinical trial with elderly women given 1 of 7 possible oral vitamin D3 doses or placebo [39]
It was found that medium dosing (1600–3200 IU/per) showed the greatest reduction in falls compared to low (400–800 IU/day) or high dosing (4000–4800 IU/day). Interestingly, this study found that fall rates increased in patients with serum 25 OHD levels above 40–45 ng/mL, although the reason for this remains unclear. Duque et al. showed that vitamin D supplementation appeared to improve muscle strength and decrease falls in older patients who have low baseline levels of 25 OHD (< 30 nmol/L) [40]. Antoniak et al. conducted a meta-analysis relating vitamin D supplementation and falls in older adults found that supplementation of vitamin D decreased the risk of falls in older individuals when combined with resistance exercise [41]. In summary, vitamin D supplementation appears to be beneficial in the elderly when baseline levels of 25 OH D are low (< 30 nmol/L).

5. Outcomes after orthopaedic surgeries

5.1. ACL reconstruction
Barker et al. studied the effects of vitamin D on strength gains after an ACL injury and subsequent reconstruction [42]. This retrospective analysis was conducted in a group of
18 Caucasian males who underwent ACL reconstruction compared to 11 control subjects without ACL injury by measuring single leg isometric strength, vitamin D, and cytokine levels 2 weeks prior to surgery and 3 months postoperative. Cytokines were elevated in the study group compared to the control group in both pre- and postoperative blood samples. Results showed that isometric leg strength increases postoperative were significantly lower in participants whose vitamin D levels were < 30 ng/mL. The authors concluded that low vitamin D levels might impair strength recovery after ACL reconstruction.
In another rat model study, Boyan et al. looked at the effects of intra-articular vitamin D injections on in vivo ACL transections [9]. Compared to placebo injections, the rats receiving vitamin D injections post in vivo ACL trans-sections showed less articular damage and lower levels of inflammatory mediators. These findings point towards some potential clinical implications on trauma induced osteoarthritis, but human clinical studies are needed to further investigate these potential correlations.

5.2. Rotator cuff injury
In a controlled laboratory study, Angeline et al. compared vitamin D deficient
rats with vitamin D sufficient control rats who underwent unilateral detachment of the supraspinatus tendon from the greater tuberosity followed by repair using bone tunnel suture fixation [43]. Results demonstrated that low vitamin D levels may negatively impact early healing of rotator cuff repair sites. It is theorized that vitamin D may have an important role in the tendon-to-bone healing process by increasing bone mineral density and strengthening skeletal muscles. However, more data is needed to determine the clinical relevance of these biochemical effects.
Oh et al. examined the relation between vitamin D levels and its effect on rotator cuff health in a retrospective study comparing vitamin D levels in patients with shoulder injuries and healthy patients with no shoulder injury [44]. The mean age of this population was 61.3 years (range, 43–80). Results showed that low vitamin D levels were associated with significant fatty degeneration of the rotator cuff muscles. Furthermore, vitamin D levels were positively correlated with isokinetic muscle torque of the shoulder.

In a clinical study evaluating the relationship of vitamin D in patients undergoing arthroscopic rotator cuff repairs for full-thickness rotator cuff tears, Ryu et al. found that low vitamin D levels were not related to outcomes after arthroscopic repair of rotator cuff muscles [45]. In this cohort, 80 patients (88%) were vitamin D insufficient, 8 (9%) were vitamin D deficient, and 3 (3%) were vitamin D sufficient.
The authors concluded that low vitamin D levels had no correlation with size of the tear, fatty infiltration in cuff muscles or the extent of the retraction, or functional outcomes as measured by the University of California, Los Angeles (UCLA) score, Constant score, and American Shoulder and Elbow Society (ASES) score at 1 year postoperatively. A major limitation of this study, however, was that only 3 patients were vitamin D sufficient. The study thus lacked power to definitely state the relationship between vitamin D and rotator cuff repair outcomes. Furthermore, the authors did not retest vitamin D levels at final follow up to adjust for variations.

5.3. Osteoarthritis
The existing literature on vitamin D levels in patients with osteoarthritis (OA) has been inconsistent. In a randomized controlled trial investigating 800 IU daily vitamin D supplementation versus placebo in 474 patients with knee OA, Arden et al. found that vitamin D supplementation did not have any effect on joint space narrowing, pain, stiffness, or functional loss [46]. This is consistent with other randomized controlled trials demonstrating no reduction in knee pain or cartilage volume loss in patients with symptomatic knee OA and low vitamin D levels [47][48].
In contrast, Zheng et al. found that vitamin D sufficiency had a beneficial effect on cartilage loss, effusion-synovitis, and physical function as measured by the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) [49].
A randomized controlled trial by Sanghi et al. demonstrated a small but statistically significant benefit of vitamin D use in patients with knee OA and vitamin D insufficiency as measured by the visual analogue scale (VAS) and WOMAC [50]. Levinger et al. found that patients with knee OA and vitamin D insufficiency (≤ 50 nmol/L) had worse knee function during balance recovery, increased pain, and locomotor dysfunction as measured by the WOMAC [51].

Similarly, Manoy et al. found that vitamin D supplementation improved quality of life, handgrip strength, physical performance, and decreased pain and oxidative protein damage in patients with OA, as measured by the WOMAC and VAS scales [52].
Several meta-analyses of vitamin D and OA have demonstrated significant associations between low vitamin D levels and knee OA, including joint space narrowing and structural changes in the knee [53][54] Regarding vitamin D supplementation in knee OA, a meta-analysis by Gao et al. found supplementation improved WOMAC pain and function, but had no effect on the prevention of tibial cartilage loss [55]. Conversely, a meta-analysis by Diao et al. found supplementation had no clinically significant effect on pain relief or joint structure improvement in patients with knee OA [56]. Despite the extensive literature on relationship between vitamin D and OA, further longitudinal studies are needed to reconcile these discrepancies in the literature.

5.4. Arthroplasty
There are several studies evaluating vitamin D status in patients undergoing arthroplasty surgeries. Shin et al. conducted a prospective cohort study with patients undergoing total knee arthroplasty (TKA) and found that early post-operative outcomes following TKA were affected by patients’ preoperative vitamin D status [57] Using a cut-off value of < 12 ng/mL to define deficiency, they found that those in the vitamin D-deficient group had significantly worse outcomes as assessed by the functional American Knee Society Score (KSS), the alternative step-test, and the six-meter walk test. 
These findings are supported by several other studies investigating vitamin D levels and pre-/post-operative functional scores in patients undergoing TKA [58][59][60]. In contrast, Lee et al. investigated vitamin D levels in patients undergoing TKA and found there was no effect of vitamin D levels on total WOMAC (p = 0.22) postoperatively, although there was an increased risk of moderate-to-severe pain in patients with low vitamin D [61]. They concluded that hypovitaminosis D was not associated with worse health-related quality of life at 3 months postoperatively following TKA.

Nawabi et al. evaluated vitamin D levels and Harris hip scores in 62 Caucasian patients undergoing THA for OA and found that vitamin D deficient patients had lower preoperative Harris hip scores (p = 0.018) and were less likely to have an excellent outcome postoperatively (p = 0.038) [62]. In contrast, Unnanuntana et al. investigated vitamin D levels and short-term functional outcomes in 219 patients following THA and found no associations between THA and functional outcomes at 6 weeks postoperatively as measured by the WOMAC, Short Form-36, 2-minute walk test, and timed get up-and-go tests [63]. Additionally, the same authors investigated vitamin D levels in a separate cohort of 200 THA patients and found no associations between vitamin D levels and in-hospital milestones, length of hospital stay, or perioperative complications [64].
In a prospective observational study by Maier et al. assessing the effect of low vitamin D levels and length of hospital stay in 1083 patients after elective hip or knee arthroplasty, patients with low vitamin D levels stayed on average 4.3 days longer post-arthroplasty than patients with adequate vitamin D levels [65]. Eighty-six percent of these patients were vitamin D insufficient, and over 60% were vitamin D deficient. Future randomized controlled clinical trials are necessary to clarify the conflicting evidence in the current literature regarding vitamin D and outcomes in total joint arthroplasty.

Recent studies have investigated postoperative complications associated with low vitamin D levels in patients undergoing arthroplasty procedures. Traven et al. retrospectively evaluated 126 revision TJA patients and found a prevalence of 55% vitamin D deficiency in this population [66]. They found that low vitamin D was associated with an increased risk of 90-day complications, including an increased risk of periprosthetic joint infection as the reason for revision surgery. These findings are supported by a study by Maier et al. [67]. A high percentage of patients requiring revision surgeries due to aseptic loosening (52%) and periprosthetic joint infection (86%) had vitamin D levels < 20 ng/mL. 
These results suggested an association between periprosthetic joint infection and vitamin D deficiency. Using an intra-articular knee implant mouse model, Hegde et al. found that vitamin D deficient mice had increased bacterial burden and neutrophil infiltration at the knee joint [68]. Furthermore, they found that replenishing the vitamin D deficient mice with supplemental vitamin D3 could reverse this effect. Further studies are needed to investigate the potential association between low vitamin D levels and increased risk of periprosthetic joint infection. It’s back to baseball season, as the Ichabod Crane Middle School modified team is gearing up for its first games of the season. Coach Bob Hanna said the team is starting off strong.

What You Need To Know:

Kyle Stickles returned to the baseball field after being diagnosed with a rare form of bone cancer in 2020. He went through months in the hospital receiving chemotherapy after he was diagnosed with osteosarcoma, had surgery to remove five inches of bone and had to relearn how to walk.

As a lifelong Jets fan, he says support from fans all over the country helped him through his toughest days. The teen will be announcing the first-round NFL Draft pick for the New York Jets through Make-A-Wish. Kyle Stickles is strapping on his glove for his big return.

The 13-year-old just got the medical clearance from his doctors to get back into the outfield. “We only had one game so far, but we won it 12-6,” Hanna said. “So, we’re looking forward to the rest of the season.

“The team has really rallied around him and made it a home for him,” Hanna said.
“So, it’s really good to see him back out on the field.” It was baseball season 3 years ago when everything came to a stop. In 2020, the world shut down as it battled a pandemic. But, it was also a time when Stickles began his own battle.

Spectrum Local News shared details on Stickles’ condition back in 2021.
At just 11 years of age, he was diagnosed with osteosarcoma, a rare form of bone cancer. The journey began with a harmless trip to the doctor to help diagnose pain that Kyle, an avid baseball player, had been experiencing in his leg. It ended with a diagnosis that shook the Stickles family to their core.
Kyle spent 97 days in the hospital at Albany Medical Center, went through months of chemotherapy, had surgery to remove five inches of bone and thankfully, was given the news that he was in remission. Then in 2021, he took his first steps toward recovery.
“That’s all he really wants is to be a normal kid again,” said his mom, Jamie Stickles. Today, running the bases are some of the biggest strides of Kyle’s journey.
“And I haven’t been playing baseball in a while, and I’ve really been
wanting to play again,” Kyle said. “So, it feels great.”

Hanna said Kyle returned back to practice ready to put in the work for the season.
“He’s been dealing with things that no other kid has to deal with,” Hanna said.
“So he’s very resilient about it and doesn’t complain about anything.” But it’s his passion for sports that helped him through the toughest days. The reminders of that fight can be seen in every corner of his bedroom.

The New York Jets football team, Jets fans and sports fans across the country sent Kyle mementos of encouragement during his battle with cancer. His lifelong love for the Jets is going to be on full display on Thursday, as Kyle will be announcing the first-round pick for the team during the 2023 NFL Draft. It’s an opportunity he received through Make-A-Wish. “Anybody who knows him knows this is a perfectly Kyle wish,” Jamie said.

“Probably someone on offense,” Kyle said with a laugh when
asked if he had any insight on who the Jets may be choosing. But, no matter who the
Jets select this year, he has the support of his fellow fans and teammates behind him — just as he has throughout his entire journey. “Absolutely I’ll be watching the draft and seeing where he’s at there. So hopefully he picks a winner,” laughed Hanna.

Tonight, a boy from Columbia County will be taking to the stage to help the New York Jets announce their first-round draft pick. 13-year-old Kyle Stickles has been given the opportunity through the Make-A-Wish Foundation.
 In 2020, Stickles at the age of 11 was diagnosed with Osteosarcoma — a rare bone cancer that affects just 400 kids each year. His last chemotherapy treatment was in June 2021. 

“He worked so hard,” said his mother, Jamie Stickles.

Stickles was given the opportunity to announce the Jets pick thanks to the Make-A-Wish Foundation, Stickles was diagnosed with bone cancer when he was 11. 

Kyle Stickles may have made the biggest impression of anyone at the 2023 NFL Draft on Thursday. Stickles is not a player, coach nor general manager, but rather an extremely passionate New York Jets fan who, through the Make-A-Wish Foundation, had the opportunity to announce his favorite team’s first-round pick from the stage in Kansas City. 
The New York Jets selected Iowa State edge Will McDonald IV with the 15th pick in the 2023 NFL Draft. But that wasn’t the highlight of the night for the Jets. 

The highlight was the person who announced the pick and officially welcomed McDonald to the Jets. Typically, commissioner Roger Goodell makes the call in the first round. But the Jets went and did a really cool thing by granting a wish to 13-year-old Kyle Stickles through the Make-A-Wish program. 
Stickles, who was diagnosed with bone cancer in 2020, got to live out every fan’s dream of being up on the draft stage and announcing a team’s pick. But Stickles did more than just announce the Jets’ pick. He left a lasting impression. With the Jets’ pick in and Roger Goodell introducing him to announce the selection, Stickles threw his arms in the air screaming “Let’s Go! Let’s Go!”

When he confidently stepped behind the microphone,
Stickles enthusiastically announced, “With the 15th pick in the 2023 NFL Draft, the J-E-T-S, Jets! Jets! Jets! select Will McDonald, linebacker, Iowa State. Let’s Go! Yeah! Let’s Go!” The fired-up youngster immediately became a viral sensation on social media. Jets center Connor McGovern was one of many who appreciated his enthusiasm: In fact, you didn’t need to be a Jets player nor fan of the team to appreciate Kyle’s passion.  
Stickles was diagnosed with bone cancer in his left leg in 2020 and received chemotherapy until June 2021. He regained the ability to walk after extensive physical therapy.
He appeared at a press conference with Jets coach Robert Saleh and general manager
Joe Douglas after the Aaron Rodgers trade earlier in the week. He continued to make the most of life with his special moment Thursday at the 2023 NFL Draft. As for the selection,
New York adds to its already loaded defensive group with the McDonald selection.
